- Description of the Medicine: Including the generic and brand names, strength, and dosage form of the medicine.
- Quantity of the Medicine: The amount to be imported should be for personal use only, indicating no intent for resale.
- Prescription from a Licensed Medical Practitioner: A letter or prescription from a qualified doctor in Uganda, stating the necessity of the medicine and the lack of local alternatives.
- Proof of International Approval: Evidence that the medicine is approved in other countries and is being used appropriately.
